To complete this process digitally, it is necessary to follow five steps. First, you must log in to Mi ANSES with your CUIL and Social Security Key. Then, if information is missing from the booklet, you can generate, download, or send the form via email. Once the booklet is completed, it must be printed on acceptable quality paper and taken to the relevant school or health center to be completed and signed.
Subsequently, it is necessary to upload the completed booklet to the system. Some guidelines regarding the quality of the photo must be followed, such as choosing a flat and well-lit surface, ensuring that all four corners of the form are visible, among others. Finally, upon finishing the process, you must log back into Mi ANSES to finalize the submission.
If you have difficulties with the online upload, the booklet can be submitted physically at the office nearest to your home. It is important to remember that it must contain information about education, health, and vaccination.
